A timing problem. The money was lost to a date, not to a decision, which also makes it the easiest of the four to prevent next time.
A price went up and you noticed late
A visibility problem. The rise was announced; the announcement was one e-mail among hundreds.
Paid for something you never used
The most common of the four, and the one the European survey put a number on.

A plumbing problem — two billing routes that cannot see each other.
